Forms & Reporting

The Bonner Foundation has developed three workbooks specifically for those campuses that are utilizing the Bonner AmeriCorps Education Award Program. Hard copies of these workbooks will be mailed to your campus at the beginning of each program year. These workbooks should be used in conjunction with the Bonner Web-Based Reporting System. Below you can download PDF versions of the workbooks to review and print out if necessary:

  • Bonner AmeriCorps Orientation Workbook: contains background information on the Bonner Leaders Program and how it is connected to the AmeriCorps program.
  • Bonner AmeriCorps Enrollment Workbook: contains all the forms that need to be signed by the member, the campus administrator, and their site supervisor -- all of which need to be copied (with the original mailed to the Bonner Foundation and the copy kept on campus in the member's Bonner AmeriCorps file).
  • Bonner AmeriCorps Service Workbook: contains the Community Learning Agreement and copies of the Hours Log form. The Community Learning Agreements for each Bonner AmeriCorps member need to be copied (with the original mailed to the Bonner Foundation and the copy kept on campus in the member's AmeriCorps file). The Hours Log form can be copied and used for reporting service and training hours, but the hours must also be entered on BWBRS; regardless of which approach you choose, you must send signed, original Hour Logs to the Bonner Foundation each month for every Bonner AmeriCorps member (with copies kept on campus in the Bonner AmeriCorps member's file).
  • AmeriCorps Exit Form (pdf)

The BWBRS will also track the specific status of an AmeriCorps members enrollment status, including the enrollment date, status of each required enrollment form, their total hours completed (data that will be entered on BWBRS by the student or your staff), and their exit information. This will allow for the campus administrator and the individual Bonner AmeriCorps member to track their status in the program.

Education Award Info

Once the member has submitted the completed paper work (including signed time logs, activity reports, and exit form) the Bonner Foundation will process the exit with the Corporation for National & Community Service. At the end of each day the CNCS uploads all the exit forms. From that point it will take about 4 weeks (sometimes less) for the member to receive his/her voucher in the mail.   The Ed Award voucher will be mailed to the address noted as "Permanent Address" on the exit form.

If the member does not receive the voucher, it is best to call the Foundation (609-924-6663) first to be sure that the exit has been processed.   Someone at the Foundation will confirm the exit date, the Social Security number, and the address the voucher was mailed to. If any of that is incorrect, call or e-mail the CNCS. Once the exit has been entered in our system, the Foundation does not have access to change information.

If the exit has been processed at the Foundation, the member has lost the voucher, and/or if there are specific questions regarding their account we recommend members:

  1. Check the status of their account on-line account
  2. Download a replacement voucher ( www.americorps.org )
    In section titled Member/Alumni Info, Education Award, Check Account Status members have the option to sign up for a login & pin. Members will be able to download a new voucher as well as check the account status of their trust
  3. Call or e-mail the CNCS. (We have found that students get a faster response when emailing.) When leaving a message or sending an e-mail, be sure to include the member's SS number.
