[1] 2004-2005 Reporting Calendar
There are a variety of reports that need
to submitted to make this program work. Please mark your
work calendar with the key reporting dates on the calendar
below.
[2] Member Enrollment & File Management
The NJ State Commission and the Corporation
for National & Community Service require specific documentation
for Bonner Leaders enrolled in the NJ BLP. The Member File
Checklist will note all the required documents. The original
copy of any form requiring a signature must be kept in the
official member file held by the Program Director. We suggest
that each site keep their own copy of the documents in the
member file.
The documents that a member must provide
are as follows:
- Copy Proof of Citizenship. (Birth Certificate,
valid passport, permanent resident documentation.)
- Copy of Social Security Card
- Copy High School Diploma/GED equivalent
or College Transcripts
- State Police Record Release or complete
Background Check (only if primary site requires member to
work with children.)

2004-2005 Time Sheets
All Bonner Leaders must fill out time
sheets on a regular basis. Completed time sheets must be
signed by the site supervisor, campus coordinator, and the
member every two weeks.
Campus coordinators should send the original
copies of the signed time sheets to Joan Horton at the
Bonner Foundation, 10 Mercer Street, Princeton, NJ 08540
by the end of the first week of the following
month (i.e., September's time sheets would be mailed to
her the first week of October).
[4] Reporting on Objectives
Reporting for the NJ BLP AmeriCorps grant
is somewhat detailed. Our members work hard to not only
accomplish but to surpass our eleven service objectives.
One of our challenges is to capture all that our members
achieve in a given program year. The forms available below
document some of the specific numbers the program is required
to report on. Supporting documentation such as agendas,
sign-in sheets, photos, fliers, etc. should be mailed to
the Program Director along with the each of the reports.
For those Bonner Leaders not funded by
AmeriCorps, The NJ BLP does not need the extensive enrollment
documentation required for AmeriCorps members. However,
we would like to know about the work that all Bonner Leaders
are doing in the program. Please include their activities
in reporting (noting as volunteers recruited when appropriate)
and on Corps Rosters. We do not need hour logs for non-AmeriCorps
Bonner Leaders.
