Proposal Guidelines
Please note grant applications, whether submitted
for the first time or for renewal, must follow
the format of these guidelines to be considered
for funding.
I. Cover Page
Please fill out the enclosed cover page form found on the
link to the left ("Grant Cover Page") and attach
it to the front of your proposal.
II. Narrative
The grant application's narrative should be
a maximum of four pages.
A. Background on Sponsoring Congregation(s)
Describe the sponsoring congregation(s), addressing
each of the following:
- Its history and mission;
- Its denomination, number of members, and
outreach programs;
- Whether the hunger program is a project
of the congregation(s) or a separate, 501(c)
3 non-profit organization;
- The amount of direct financial support
to the food program by the congregation(s);
- The number of members volunteering with
the food program;
- Whether or not the congregation(s) provides
physical space and/or other in kind support
to the food program
B. Background on Hunger
Program
Please describe the proposed food program,
including each of the following:
- Its history and mission;
- A profile of the population that the agency
serves including gender makeup, age range,
and special needs of the clients;
- The areas economic conditions as
they relate to the needs the program serves;
- Key staff, their qualifications, and their
role in the food program;
- The food programs relationships
with other organizations serving the same
population.
C. Funding Request
Please provide the following:
- A monetary request and detailed budget
for the proposed program;
- A description of why the agency is requesting
this grant, what outcomes it hopes to achieve,
and how the funds will be spent if a grant
is awarded.
III. Appendix
In addition to the proposal narrative, include:
- The organizations financial statements
for the most recent fiscal year and the
current fiscal years operating budget;
- A list of the names, addresses and contact
persons for three current funding sources;
- A list of the members of the Board of
Directors, along with their professional
affiliations;
- A copy of the most recent IRS letter indicating
tax-exempt status;
- Any printed materials describing the congregation
and its outreach programs.
IV. Survey
Complete and return the enclosed survey.
