Evaluation and Recognition

 

1. Introduction to Partner Evaluations: guidelines and a sample for introducing the evaluation expectations and process to community partners, which may be introduced in the binder or in a letter with the evaluation forms.

2. Sample Bonner CLA Review Forms including:

These may be used as the standard forms, and/or you may want to consider the more detailed evaluations below.

3. Partner Evaluation of Student: Level 1-2: a two-page rating form evaluation for a partner to complete about the student, addressing job performance, working with people, professionalism and maturity, and open-ended questions about the student’s strengths and areas for improvement.

4. Student Evaluation of Partner: Level 1-2: a two-page rating form evaluation for a student to complete about the partner supervisor/agency, addressing organization, training, supervision, guidance, support, and development, and open-ended questions about recommending this placement for the future.

5. Partner Evaluation of Student: Level 3-4: a two-page rating form evaluation for a partner to complete about the student, addressing job performance, communication, attitude, working with people, responsibility, and leadership and open-ended questions about the student’s performance and development.

6. Student Evaluation of Partner: Level 3-4: a two-page rating form evaluation for a student to complete about the partner, addressing job structure, communication, attitude, responsibility, and leadership and open-ended questions about the partner’s investment in the student’s work and development.

7. Partner Recognition Key Ideas: a one-page description of key partner recognition ideas including fairs, senior presentations, awards, speaking opportunities, trainings, and ongoing communication.

8. Partner Recognition Form: a sample recognition form for use by students/programs to recognize partners.
